29 CFR 1926.451(c)(2)(v):     Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the forks:    A) Front of building, on or about June 21, 2012, a wooden platform being used on a Sky-Trak forklift to lift employees in the air was not secured to the forks, exposing the employee to a fall hazard.     ABATEMENT CERTIFICATION IS NOT REQUIRED.
