29 CFR 1926.300(b)(5): "Exposure of blades." When the periphery of the blades of a fan is less than 7 feet (2.128 m) above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than 1/2 inch (1.27 cm). a) On or about June 19, 2012; on the south side of the jobsite, employees were allowed to utilize a centrifugal fan with unguarded blades at floor level.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a) On or about June 19, 2012; on the south side of the jobsite, employees were allowed to use an electrical extension cord that was not effectively grounded. The electrical extension cord was missing the ground pin.
