29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    At the site, employees perform maintenance functions (belt chang out and electrical) and part change out (set-up for parts and chuck changes) on HEALR inside/outside diameter grinders and Springfield vertical and surface grinders.

29 CFR 1910.179(j)(3):     A complete periodic inspection of crane(s) had not been conducted in the past 12 months:    At the site, a Saturn 5 Ton overhead gantry crane used to load parts into the ATISON surface grinder and the SPRINGFIELD vertical grinder, was installed approximately 3 years ago and has not been inspected on a yearly basis.

This was observed at;    a) The HEALD ID Grinder (#2) on east wall;    b) The HEALD ID Grinder (#3) on east wall;    c) The HEALD ID Grinder (#4) on east wall.
