29 CFR 1926.51(f)(1): The employer shall provide adequate washing facilities for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be in near proximity to the worksite and shall be so equipped as to enable employees to remove such substances. a. On the site where an employee was using mortar which contained the chemical Portland Cement did not provide clean water, clean towels and non alkaline soap thereby exposing the employee to a contact hazard.

Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. On the site where extension cords were used to supply power to portable tools, the employer ran the extension cords accross the parking lot to the building under construction where the extension cords were subject to physical damage from vehicles and equipment thereby exposing employees to a contact hazard.

On the concrete wall of the second level where an employe was installing metal bolts in the concrete block walls, fall protection was not provided thereby exposing the employee to a fall hazard.
