29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. a. At the site, an employee was operating a forklift in a manner that was not designed by the manufacturer. The forklift was used to elevate personnel in a work platform at a height greater than 11-1/2 feet, thereby exposing an employee to a fall hazard.

29 CFR 1926.602(c)(1)(viii)(A): Whenever a truck is equipped with vertical only, or vertical and horizontal controls elevatable with the lifting carriage or forks for lifting personnel, the following additional precaustions shall be taken for the protection of personnel being elevated: Use of a safety platform firmly secured to the lifting carriage and/or forks. a. At the site, an employee was installing outdoor insulation (Styrofoam) from a rough terrain forklift to lift personnel in a work platform that was not secured to the forks or the lifting carriage of the forklift, thereby exposing the employee in the work platform to a fall hazard greater than 11-1/2 feet.
