29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure:     a) Kitchen Basement Doorway, on or about 7/2/12; An extension cord was being used in lieu of permanent wiring and was plugged into an outlet next to the sink, ran over the doorway and to the rice warmer.    b) Kitchen Walk-in cooler/office, on or about 7/2/12; An extension cord was being used in lieu of permanent wiring and was plugged into an outlet with a missing knock-out enclosure, around the corner and into the office doorway to power the security camera system and paper shredder.
