At the site, employees were installing vinyl siding on the side of a residential home from a pump jack scaffold that had significant damage to its work platform, thereby exposing the employees to a fall hazard of approximately 14 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. At the site, employees were installing vinyl siding on a residential home from a pump jack scaffold that did not have a complete guardrail system installed on both ends of the scaffold, thereby exposing the employees to a fall hazard of approximately 14 feet.

29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they shall be installed between the top edge of the guardrail system and the scaffold platform. a. At the site, employees were installing vinyl siding on the side of a residential home from a pump jack scaffold that did not have mesh secured from the top edge of the guardrail system and the scaffold's platform across the entire front of the scaffold, thereby exposing the employees to a fall hazard of approximately 14 feet.

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled. a. At the site, employees were installing vinyl siding on the side of a residential home from a pump jack scaffold that was not braced at the bottom of one the scaffold's poles, thereby exposing the employees to a fall hazard of approximately 14 feet.
