OSH ACT of 1970 Section (5)(a)(1):   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrocution while tacking shelter in ungrounded wooden shack(Rest rooms) during a lightning storm. Job site: Lake Of Isles golf course, 15 ground maintenance employees sustained injuries from lightning strikes while taking shelter in a wooden shack that was not provided with any means to deflect or absorb lightening. Two employees sustained second degree burns to part of their body.

Maintenance Garage storage room: The employer did not insure that Oxygen cylinders in storage were separated from fuel-gas cylinder (Acetylene Cylinder).
