Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about June 8, 2012, during commercial low slope roofing activities the employer did not ensure employees were protected from fall hazards by ensuring a guardrail system, safety net system, personal fall arrest system, or a safety monitoring system was being used. CLASSICAL CONSTRUCTION LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311608384, CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON MAY 8, 2009 WITH RESPECT TO A WORKPLACE LOCATED AT 474 STATE ROUTE 58, SULLIVAN, OHIO 44880.
