29 CFR 1910.38(c):     The employer did not implement an emergency action plan which included at a minimum the elements required by paragraphs 1910.38(c)(1)-(6) of this part:     (a)     At the All-States Equipment facilities an emergency action plan had not been implemented in order to provide for           safe evacuation and accounting of employees in the event of an evacuation type emergency. 29 CFR 1910.132(d)(1):     The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:    (a)     At the facility PPE had been provided and required to be used in areas such as but not limited to welding and           wood working.  A hazard assessment had not been performed to determine if hazards were present which             necessitate the use of personal protective equipmsnt.
