29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports : a) construction site, 370 Chestnut St., Meadville, PA. On or about June 27, 2012, sections of the tubular welded frame scaffold were not fully planked. These sections included but were not limited to the top section of the frame scaffold located across from the top of buildings turret. The scaffold deck was positioned approximately 30-feet above ground level.
