OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that PVC plastic was used to distribute compressed air throughout inside the print shop to equipment such as the Miller Perfector printing press:    a)  On or about 6/27/12 employees working near the Miller Perfector printing press were exposed to being struck-by shards of PVC plastic if the pipes explode.  The piping was used to deliver compressed air @ 110psi to equipment such as the Miller Perfector printing press.   The PVC pipe was interconnected to air compressors such as a Speedaire.    Among other methods, feasible and acceptable abatement methods to correct this hazardd are:    1.  To replace the pipe with an approved ABS plastic  2.  To encase the PVC pipe within shatter resistant materials  3.  To use compressed air hoses

29 CFR 1910.37(b)(6):  Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:    a)  On or about 6/27/12 throughout the press shop exit signs were not illuminated or self-luminous.  Paper copies of exit signs were used to designate exits.

29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)  On or about 6/27/12 at 2108 Whitfield Park Loop, Sarasota, FL the employer did not establish a written lockout tagout program for tasks such as but not limited to changing printing press rollers, cleaning grippers, and greasing fittings of equipment such as the Miller Perfector, Roland Favor It, and Bestech 40.
