29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects:    On or about June 27, 2012, employees were exposed to struck-by hazards when the employees who were bringing bricks and mortar to the base of the scaffold and using the rope and pulley to pull buckets of bricks and mortar to the top of the scaffold were not wearing hard hats.  In addition, scaffold frames were also being pulled up to the top level in order to increase the height of the scaffold.

Crossbraces were used as a means of access.    On or about June 27, 2012, employees were exposed to fall hazards of approximately 22 feet when the employees were working on a scaffold that did not have a ladder to permit safe access to the upper levels of the scaffold.
