29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts shall be guarded in accordance with the specific requirements of 1910.219.    a) RM Clark Inc. located on 400 Crystal Avenue in Findlay, OH:  On or about June 27, 2012, the employer did not assure the rotating chain on the conveyor of Notcher machine #1 was guarded.  Employees were exposed to hazards created by the rotating parts and ingoing nip-points.    b) RM Clark Inc. located on 400 Crystal Avenue in Findlay, OH:  On or about June 27, 2012, the employer did not assure the rotating chain on the conveyor of Notcher machine #2 was guarded. 29 CFR 1910.332(b)(1): Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.    a) RM Clark Inc. located on 400 Crystal Avenue in Findlay, OH:  On or about June 27, 2012, the employer did not ensure a maintenance employee who performed troubleshooting and testing activities with a Sperry DM-5300 multi-meter routinely on Notcher machine #1 (480V) was provided with training to protect him from electrical shock, arc flash, and arc blast hazards.     b) RM Clark Inc. located on 400 Crystal Avenue in Findlay, OH:  On or about June 27, 2012, the employer did not ensure a maintenance employee who performed troubleshooting and testing activities with a Sperry DM-5300 multi-meter routinely on Notcher machine #2 (480V) was provided with training to protect him from electrical shock, arc flash, and arc blast hazards.      c) RM Clark Inc. located on 400 Crystal Avenue in Findlay, OH:  On or about June 27, 2012, the employer did not ensure a maintenance employee who performed troubleshooting and testing activities with a Sperry DM-5300 multi-meter routinely on Notcher machine #3 (480V) was provided with training to protect him from electrical shock, arc flash, and arc blast hazards.

The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards:    a) RM Clark Inc. located on 400 Crystal Avenue in Findlay, OH:  On or about June 27, 2012, the employer did not ensure that a maintenance employee working on 480 volts, 3-phase energized control circuits followed safe work practices.  The supervisor was not aware of safe approach distances or the need for electrical personal protective equipment when servicing equipment such as, but not limited to Notcher machine #1.
