29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls was acquired by employees.     a. Ohio Metallurgical Services, Inc. located in Elyria, Ohio:  On or about June 11, 2012, the employer did not ensure the operator for furnace 3203 was properly trained as an authorized employee under the lockout standard.  The operator and a maintenance employee were unjamming the furnace using a large hook when the flammable atmosphere inside of the furnace ignited causing an explosion which burned the employees performing the unjamming activity.  Neither employee locked out the furnace prior to starting the unjamming activity.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown was not utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.    a. Ohio Metallurgical Services, Inc. located in Elyria, Ohio:  On or about June 11, 2012, the employer did not ensure furnace 3203 was properly shut down prior to employees performing unjamming activities.  As employees were using a long hook to unjam the transfer chain and parts rack inside of the furnace, the flammable atmosphere inside of the furnace ignited causing an explosion which burned two employees performing the unjamming activity.
