29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.    The rails of an extension ladder did not extend a minimum of 3 feet above the work surface that employees were accessing and egressing.    There is no abatement certification or documentation required for this item

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    The employer did not provide a fall protection training for his employees that were exposed to fall during residential roofing operations.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c)
