29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    a) Small Parts 2nd Floor Storage - On or about 6-28-12 - Storage area was missing mid rails and toeboard above the stairway, exposing employees to fall hazard of approximately 8 feet.    b) Dismantling Area - On or about 6-28-12 - Storage area approximately 8 feet above the floor was not equipped with a standard railing system, exposing employees to fall hazards.

29 CFR 1910.23(d)(1)(ii):     Stairway(s) less than 44 inches wide having one side open, did not have at least one stair railing on the open side:    a)  Small Parts Stairway - On or about 6-28-12 - Employees were exposed to fall hazard of 5 to 8 feet from wooden stairway that was not equipped with a standard railing system.  There was no mid-rail installed on the fight of stairs to the second floor storage area.

29 CFR 1910.110(e)(4)(iii):     Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating:    a) At the facility - On or about 6-28-12 - Removable LPG (liquefied petroleum gas) fuel cylinder mounted on the back of the Toyota Fork Lift did not have the cylinder properly installed on the safety pin which would stop the cylinder from rotating while the fork lift was in operation.

29 CFR 1910.178(l)(1)(ii):   The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.    a)  At the faciltiy - On or about 6-28-12 - Employees were allowed to operate industrial fork lifts without being evaluated or trained by the employer.  Employees were operating Toyota and Clark fork trucks.
