29 CFR 1910.141(a)(5): Enclosed workplaces were not so constructed, equipped, and maintained as to prevent the entrance or harborage of rodents, insects, or other vermin: For work and break areas within the facility: On or about June 27, 2012, and at times prior thereto, the employer did not ensure that work and break areas in the facility were kept free of insects, exposing employees to unsanitary conditions. Specifically, live crickets were entering the workplace, and dead crickets were not promptly removed. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure insects are being kept out of the facility.

29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: For material handling performed inside the facility: On or about June 27, 2012, and at times prior thereto, the employer did not ensure that the all alloy steel chains used in the facility had identification tags stating the size, grade, rated capacity and reach, exposing employees to being crushed by falling loads in the event of overloading the chain. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all alloy steel lifting chains have a permanently affixed durable identification stating size, grade, rated capacity, and reach.
