29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     On or about June 28, 2012, during construction activities, the employer allowed employees to work on a roof that was approximately 10 feet high with an 8/12 pitch and did not ensure that the employees were protected from falls.    NOTE: TRI-STAR HOME IMPROVEMENT INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 1926.501(b)(11), WHICH WAS CONTAINED IN THE OSHA INSPECTION NUMBER 313784167, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 14, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 1201 EAST MALL DRIVE, HOLLAND, OH 43528.
