29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a)  City Dry Cleaning, Inc., Findlay, Ohio:  On or about June 28, 2112, the employer did not ensure that employees who were exposed to physical contact with hot surfaces of equipment, such as, but not limited to the shirt press, the cuff press, the puffer and the buck machine, were protected with appropriate protective equipment for the arms, exposing them to burn hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) City Dry Cleaning, Inc., Findlay, Ohio:  On or about February 13, 2012, the employer did not ensure that the point of operation on the American Laundry Machinery, Inc. manufactured shirt press (Model 451-C, S/N 292334390188) was guarded to prevent employees who were working in the area from inadvertently accessing the danger zone while the machine was in operation.  An employee was injured when the machine operator cycled the machine and the nearby employee had their hand inside of the press, resulting in 3rd degree burns.    b) City Dry Cleaning, Inc., Findlay, Ohio:  On or about June 10, 2012, the employer did not ensure that the point of operations on the American Laundry Machinery, Inc. manufactured cuff presses were guarded to prevent employees who were working in the area from inadvertently accessing while the danger zone while the machine was in operation.  Employees working in the area were exposed to burn hazards.
