29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used.     (a) Location - 2 Gregory Circle, Andover, MA:  On or about 6/26/2012, employees were climbing off of a roof and onto to a wall bracket scaffold an then jumping into an open window to get off of the roof.    Dawn Melanson Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.451(e)(1) which was contained in OSHA inspection Numbers 316312149, Citation Number 1, Item Number 001(a), Issued on 3/21/2012 with a final order date of 4/12/2012 with respect to a workplace located at 41 Primrose Street, Haverhill, MA.
