29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Roll Mixer - Maintenance employees and the Chief Engineer were exposed to a fall of at least 7 feet, 9 inches from the roll mixer to the floor below. The exposures occurred while they were removing flour from the inside walls of the flour hopper.
