(b) Second floor room 245: On or about June 22, 2012 at 1005hrs, the exit pathway was reduced to less than 19 inches by the placement of furniture in the office; the exit pathway minimum is 28 inches.

(b) Second floor room 240 B-1: On or about June 22, 2012 at 0959hrs, a relocatable power tap was used to provide 110VAC service to a space heater. (c) Second floor room 245: On or about June 22, 2012 at 1007hrs, a relocatable power tap was used to provide 110VAC service to a refrigerator. (d) First floor exit hallway: On or about June 22, 2012 at 1016hrs, a relocatable power tap was used to provide 110VAC service to a microwave oven. (e) Fourth floor conference room 445: On or about June 22, 2012 at 1525hrs, a relocatable power tap was used to provide 110VAC service to a microwave oven.
