29 CFR 1918.22(b):     Each side of the gangway, and the turntable if used, did not have a hand rail with a minimum height of 33 inches (0.84 m)(measured perpendicularly from rail to walking surface at the stanchion) and a midrail.    a) Dock Gangway - On or about 6/27/12, the gangway did not consist of a top-rail and a mid-rail, exposing employees to fall hazards.
