Safety of equipment shall be determined on the basis of the following considerations:    (a).  Worksite located at 601 NW 40 Highway, Blue Springs, Missouri- Employees were exposed to a shock or electrocution hazard in that the plug located on the scissor lift had its' outer insulation pulled away, thus exposing the inner wires of the cord.
