29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    (1) On the west side of the construction site, an employee was allowed to step on a wood pallet that was being supported by a Pettibone forklift without fall protection to offload concrete blocks.  The pallet was elevated more than 25 feet above the next lower level, exposing the employee to a fall hazard.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (1) On the west side of the construction site, an employee working from the top of a concrete wall, 25 feet above the next lower level, was not protected from falling as no fall protection measures were in place to protect the employee from falling while offloading concrete blocks.

29 CFR 1926.602(c)(1)(vi):     Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.    (1) On the west side of the construction site, a Pettibone forklift supporting a wood pallet with concrete blocks was left unattended with the load elevated more than 25 feet above the next lower level, exposing employees in the area to struck-by and crushed-by hazards.    (2) On the west side of the construction site, a GEHL forklift was left unattended with the engine running, exposing employees in the area to a struck-by hazard.
