29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about June 26, 2012 - On the shop floor, OKUMA Cadet, Serial #0218 was being operated with the interlocks disconnected.    b) On or about June 26, 2012 - On the shop floor, OKUMA Cadet, Serial #0592 was being operated with the interlocks disconnected.    c) On or about June 26, 2012 - On the shop floor, OKUMA Cadet-Mate #3 was being operated with the interlocks disconnected.    d) On or about June 26, 2012 - On the shop floor, four Alliant milling press had unguarded rotating parts.    e) On or about June 26, 2012 - On the shop floor, a ChunJei 13 inch automatic horizontal band saw, Model #C330W, Serial #CJ331060, was being operated without a guard over the left side of the blade.     f) On or about June 26, 2012 - On the shop floor, a manual horizontal milling lathe, Model,#S2100, Serial #712192, had a unguarded rotating part.    g) On or about June 26, 2012 - On the shop floor, a manual horizontal milling lathe, Model #S20120, Serial #714169, had an unguarded rotating part.

29 CFR 1910.1200(f)(5)(i):     The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:    a) On or about June 26, 2012 - South wall at the grinder, a cup was not marked.     b) On or about June 26, 2012 - North wall next to an electrical blower, a coffee can was being used to collect oil from the blower, which contained oil that was not marked.    c) On or about June 26, 2012 - Behind the VF-7 Vertical milling machine, a green container was being used to collect oil from the VF-7 milling machine that was not marked.     d) On or about June 26, 2012 - Behind the OKUMA miiling machine, a bucket and coffee can were being used to collect chemicals that were not marked.    e) On or about June 26, 2012 - On the Alliant milling machine, a spray bottle was being used with chemicals that was not marked.
