29 CFR 1926.501(b)(4)(ii):     Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:    On or about June 20, 2012, on a roof, where the employer did not ensure that workers were protected from tripping in, stepping into or through uncovered 4x4 mechanical shafts. Inspectors were crossing over ceiling joist assembly beams while being exposed to fall hazards of approximately 22-feet to ground below from the presence of uncovered mechanical shafts located throughout the lower level below.

29 CFR 1926.701(b):     All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  (a)(LOCATION)(IDENTIFY OPERATIONS AND/OR CONDITIONS)(IDENTIFY HEIGHT OR WORK LOCATION OF PROTRUDING STEEL)    On or about June 20, 2012, on the roof, where the employer did not ensure that employees walking/working adjacent to unprotected protruding reinforcing steel (rebar) were not protected from impalement hazards.
