Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.      a) 2301 Belle Chasse Hwy., Gretna, LA; The employer failed to ensure that three outlets connected to temporary power had covers and/or faceplates. The missing faceplates exposed employees to possible contact with live electrical conductors and the hazard of electrical shock.
