29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    At the women's dormitory roof edge on or about July 2, 2012, and at times prior thereto, eye protection was not used while performing pressure washing operations. Employee was exposed to the hazard of being struck in the eyes by debris.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    At the women's dormitory roof edge on or about July 2, 2012, and at times prior thereto, there was no fall protection for an employee who was pressure washing the roof. Employee was exposed to the hazard of falling from a height of 16.25 feet to the ground below.

At the women's dormitory roof edge on or about July 2, 2012, and at times prior thereto, the employer did not have a written certification of fall protection training.
