29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a. On or about June 25, 2012, at the above addressed jobsite, an employee was exposed to the hazards of excavation wall collapse while working in an 8.5 foot deep hole which was not protected by sloping, shoring, benching and/or shielding. R & J PLUMBING COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315508739, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 18, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 350 CENTRAL IN WILMETTE, IL.
