29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    The employer does not provide guardrails for pump jack scaffolds.  This violation was observed on or about June 11, 2012, on the east side of the residential structure where employees were exposed to a 20 foot fall hazard when working from a pump jack scaffold that did not have guardrails.

29 CFR 1926.451(c)(2):  Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    The employer does not provide base plates for supported pump jack scaffold legs.  This violation was observed on or about June 11, 2012, on the east side of the structure where employees were exposed to a 20 foot fall hazard working from a pump jack scaffold with supported legs that did not bear on base plates.    Well Constructed, LLC was previously cited for a violation of this occupational safety and heath standard or its equivalent standard 29 C.F.R. 1926.451(c)(2), which was contained in OSHA inspection number 314908583, citation number 1, item number 1 and was affirmed as a final order on December 29, 2010, with respect to a workplace located at 1550 Katy Fort Bend Rd., Katy, TX 77494.

29 CFR 1926.452(j)(2):  Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    The employer does not provide bracing to the bottom of pump jack scaffolds.  This violation was observed on or about June 11, 2012, on the east side of the structure where employees were exposed to a 20 foot fall hazard working from a pump jack scaffold that did not have bracing at the bottom of the scaffold.      Well Constructed, LLC was previously cited for a violation of this occupational safety and heath standard or its equivalent standard 29 C.F.R. 1926.452(j)(2), which was contained in OSHA inspection number 314908583, citation number 1, item number 2 and was affirmed as a final order on December 29, 2010, with respect to a workplace located at 1550 Katy Fort Bend Rd., Katy, TX 77494.
