29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about June 29, 2012, during construction activities, the employer did not ensure that employees were provided with fall protection systems/devices that enabled them to be protected from falls at all times, where exposed to fall hazards of up to approximately 50 feet. Employees were on the roof with only single lanyards and were moving from the hatch to the tie off points or from tie off point to tie off point along the roof's edge and were not able to be protected against fall hazards while doing so.
