29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:         (a) JC Construction, Inc. @ 1500 Curtis St., Denver, CO 80216: On 6/29/12, the employer did not ensure that employees were protected from flying debris while demolishing a concrete sidewalk. Employees were not equipped with eye protection. This condition exposed employees to flying debris and struck-by eye hazards.

29 CFR 1910.134(d)(1)(iii): The employer did not identify or evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminants chemical state and physical form:     (a) JC Concrete, Inc. @ 1500 Curtis St., Denver, CO 80216: On and before 6/29/12, the employer did not evaluate respiratory hazards associated with exposure to silica dust during demolition activities.     29 CFR 1926.103: The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1910.134 of this chapter.
