29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. (a) Jobsite - 7440 McKnight Road, Pittsburgh, PA: On or about June 25, 2012, two employees were observed working to tear off the roof cap near unprotected sides or edges measuring approximately 20 feet high without a means of fall protection. The employer BUH Construction Inc. was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 1926.501(b)(10) which was contained in OSHA inspection number 314105099, Citation number 1, Item number 7, issued on November 03, 2011, with respect to a workplace located at 221 Schenley Drive, Pittsburgh, PA 15217.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. (a) Jobsite- 7440 McKnight Road, Pittsburgh, PA: On or about June 25, 2012, the employer was unable to provide written certification stating the employees working at heights had fall protection training.
