29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On or about June 20, 2012, ascertained that employees are required to climb on top of recreational vehicles (travel trailers), up to approximately 15 feet in height, to repair or install air condition units, at the establishment and/or out in the field, and are not provided with some means of personal protective equipment such as but not limited to: safety harnesses with lanyards and/or scaffolds, for the prevention of falls, exposing employees to the hazard of a 15 foot fall.  In the Alterative:  Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to fall hazards of up to approximately 15 feet, when walking/working on the tops of recreational vehicles (travel trailers): The employer does not ensure that each employee working from the top of recreational vehicles (travel trailers) with unprotected sides and edges, is protected from falling by a personal fall arrest system (PFAS) or an appropriate custom-fit guardrail system. This violation was ascertained on or about June 20, 2012, for the Maintenance and Repair Shop area and/or out in the field, where employees are not protected from the hazard of a fall of up to 15 feet onto the concrete surface below. Among other methods, one feasible and acceptable means of abatement would be to ensure that employees performing work from the top of recreational vehicles (travel trailers) are protected from falls by a PFAS utilizing anchorages attached to existing overhead structure, or an equivalent means of fall protection.   Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation describing the steps that is taking to ensure that employees working from the tops of recreational vehicles (travel trailers) are protected from falls.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about June 20, 2012 at the establishment where employees are required to operate the forklifts to move recreational vehicles (travel trailers) around the yard with out being trained by the employer, exposing employees to the hazard of being struck by the forklifts or loads.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each powered industrial truck operator is trained and competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:   On or about June 20, 2012 at the establishment in the Sales Managers office, electrical circuit breaker panels were being blocked by advertisement pamphlets of Recreational Vehicles, exposing employees to the hazard of electrical shock and burns.
