29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit": Carpentry Shop, West Exit: On or about June 26, 2012 and at times prior thereto, the exit sign was not visible and did not meet the minimum stroke height and width. This condition exposed employee to smoke inhalation hazards from delay in accessing a readily visible exit.

29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches: (a) Marine Railway, Roanoke Island, Weather Deck, Fore Peak Hatch: On or about June 26, 2012 and at times prior thereto, the hatch was open. This condition exposed employees to a fall hazard of seven (7) feet. (b) Marine Railway, Roanoke Island, Weather Deck, Forward Berthing Hatch: On or about June 26, 2012 and at times prior thereto, the hatch was open. This condition exposed employees to a fall hazard of seven (7) feet. (c) Marine Railway, Roanoke Island, Weather Deck, Battery Space Hatch: On or about June 26, 2012 and at times prior thereto, the hatch was open. This condition exposed employees to a fall hazard of seven (7) feet. (d) Marine Railway, Roanoke Island, Weather Deck, Aft Steering Hatch: On or about June 26, 2012 and at times prior thereto, the hatch was open. This condition exposed employees to a fall hazard of seven (7) feet. (e) Marine Railway, Roanoke Island, Weather Deck, Aft Berthing Hatch: On or about June 26, 2012 and at times prior thereto, the hatch was open. 29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2): (a) Marine Railway, Roanoke Island, Ship's Ladder to Mess Deck: On or about June 26, 2012 and at times prior thereto, the ladder was removed. This condition exposed employees to a fall hazard of seven (7) feet to the steel deck below. (b) Sheet Metal Shop, Sheet Metal Mezzanine: On or about June 26, 2012 and at times prior thereto, the entry to the mezzanine created an open-sided floor. This condition exposed employees to a fall hazard of approximately eight (8) feet to the hard surface below.
