29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      Employees were exposed to caught in hazards while working around machines that are being repaired such as but not limited to the Accupress hydraulic press brake and the Flagler Cold Forming Machine.   A LockOut/TagOut Program would address the hazards and steps to be taken for avoiding hazards.

29 CFR 1910.212(a)(5):     Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:      Shop  Area, west side, on the pedestal fan, it did not have a guard on the front blade side, and the guard on the rear had opening several inches wide, exceeding the maximum  inch opening allowed.

29 CFR 1910.305(g)(2)(ii):     Flexible cords were not used only in continuous lengths without splice or tap:        a)	Shop  Area, west side, the 110 volt extension cord was spliced into the flexible power cord for the Skilsaw, 7 ?inch portable saw.      b)	In the Shop Area, on a 110 volt pedestal fan, the power cord was spliced into the motors wiring.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):      Shop  Area, on the orange 110 volt extension cord, it was in used supplying electricity to a pedestal fan, with damage/openings  in its outer insulation.  The outer insulation was pulled away from plug, allowing strain to be placed the inner electrical connectors.
