29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a) On or about July 2, 2012, employees were exposed to cave in hazards where a safe means was not used to determine the location of a sewer line. An employee entered a trench with unprotected vertical walls in excess of five feet and was subsequently injured when the trench walls collapsed. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
