29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:        a)  Onboard crane barge: Voids number one thru twenty:  On or about June 14, 2012 and at times prior thereto, employees were allowed to use 3M 7503 dual cartridge respirators while welding in voids and the employer had not developed and implemented a written respiratory protection program.

29 CFR 1915.12(d)(1): The employer did not ensure that each employee entering a confined or enclosed space or other areas with dangerous atmospheres was trained to perform all required duties safely:        a)  Onboard crane barge, voids number one thru twenty:  On or about June 14, 2012 and at times prior thereto, employees were welding inside voids onboard the crane barge and the employer had not ensured they were trained to perform all required duties safely.

29 CFR 1915.12(e)(2): The employer shall inform outside rescue teams of the hazards that the team may encounter when called to perform confined and enclosed space or other dangerous atmosphere rescue at the employer's facility so that the rescue team can be trained and equipped:    a)  Onboard crane barge voids number one thru twenty:  On or about June 14, 2012 and at times prior thereto, employees were working inside confined spaces and the employer had not informed the outside rescue team of the hazards they may encounter if called to rescue employees from the space.

29 CFR 1915.81(a)(1): The employer did not maintain good housekeeping practices to eliminate hazards to employees:        (a) Onboard crane barge, main deck:  On or about June 14, 2012 and at times prior thereto, the employer did not maintain good housekeeping conditions at all times by not elevating hoses and electric conductors . The condition exposed employees to tripping hazards which could result in a fall on to a steel deck.
