On or about June 28, 2012, at Bldg. #1307, Fort Buchanan, PR, employees were observed working at the very edge of a roof, with an unprotected side or edge at heights up to 24 feet from the ground level, without a conventional fall protection system, such as guardrail, safety net, or personal fall arrest.
