29 CFR 1910.23(c)(1):  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  	  a) On or about June 22, 2012 and at times prior, there was inadequate fall protection for employees working on the outbound platform that surrounds the ram system, on the north side of the shredder roof area.   The moveable guardrail and portable floor grate used in the past, when the hood is open, was not in use to address the fall hazards, when employees are accessing the roof area of the hood.  Employees are exposed to fall hazards in excess of 15 feet.      Abatement Note: Abatement certification and documentation are required for this item. 29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    On or about June 22, 2012 and at times prior, protective equipment such as fall protection was not provided to employees to prevent fall in excess of 5 feet at the following locations:    1) 2 Employees working on top the roof cover of the shredder machine making repairs to the spray bar waterline.     2) Employees working on the infeed platform access area to clean off scrap metal that had dropped off the inbound conveyor were exposed to fall hazards in excess of 10 feet on the east side and approx. 5 feet on the west side.    Abatement Note: Alternate abatement to guardrails in order to protect employees from falls maybe a "fal restraint" system, not a "fall arrest" system.    Abatement Note: Abatement certification and documentation are required for this item. (see enclosed Certification of Corrective Action worksheet).
