29 CFR 1910.37(a)(2): Exit routes must be arranged so that employees will not have to travel toward a high hazard area, unless the path of travel is effectively shielded from the high hazard area by suitable partitions or other physical barriers.    a) Facility, 9419 Route 286 Highway West, Homer City, PA: On or before 7/5/12, employees exiting building 2 through a designated EXIT route were required to travel through a high hazard area created by the storage of two propane cylinders located outside of building adjacent to the exit discharge. The propane cylinders are used to fuel a Kalmar AC forklift truck.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    a) Facility, 9419 Route 286 Highway West, Homer City, PA: On or before 7/5/12, two propane cylinders located outside of building two were not secured to prevent them from toppling. The propane cylinders are used by a Kalmar AC forklift truck operator to fuel the powered industrial truck.

29 CFR 1910.307(c):     Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location:    a) Facility, 9419 Route 286 Highway West, Homer City, PA: On or before 7/3/12, a fluorescent light switch located in the Steel Building, a Class II hazardous location, was not approved for the location. The light switch was located in 4 inch x 4 inch ordinary electrical box located approximately 4 feet off the ground adjacent to the entry door located in front of the building.
