29 CFR 1926.451(f)(6):     Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).    Note: 2 feet for insulated power lines of less than 300 volts per table (f)(6)     a) Jobsite, 2nd and Market Street, Elizabeth, PA: On or before June 27, 2012, an employee was working on an aluminum Werner ladder jack scaffold erected within 1 foot of 240 volt insulated power lines that provided power to the building.

29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) Jobsite, 2nd and Market Street, Elizabeth, PA: On or before June 27, 2012, an employee was working on a Werner ladder jack scaffold without fall protection. The employee was exposed to a 16 foot fall hazard.

29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Jobsite, 2nd and Market Street, Elizabeth, PA: On or before June 27, 2012, two employees without fall protection were removing shingles on a steep roof. The employees were exposed to a 19 foot fall hazard to the ground level.
