29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Jobsite, 2205 East Carson Street, Pittsburgh, PA: On or about June 27, 2012, during construction activities, the employer did not ensure that approrpriate fall protection systems/devices were used while workers were installing tiles to the side of a dormer on the front of the building in that the parapet wall surrounding this area was approximately 32 inches in height in at least one area, exposing employees to falls of up to approximately 17 feet.

29 CFR 1926.502(f)(1): The warning line was not erected around all sides of the roof work area:    a) Jobsite, 2205 East Carson Street, Pittsburgh, PA: On or about June 27, 2012, during roofing activities, the employer did not ensure that warning lines, used in some places for fall protection on the rear roof, were erected around the full roof area, including area by the chimney and the hoist. Guardrails that had been installed here were taken down and replaced by flags, which were also taken down and not replaced, exposing employees to falls of up to approximately 17 feet.

29 CFR 1926.502(f)(1)(i):  When mechanical equipment was not being used, the warning line was not erected not less than 6 feet (1.8 m) from the roof edge:    a) Jobsite, 2205 East Carson Street, Pittsburgh, PA: On or about June 27, 2012, during roofing activities, the employer did not ensure that warning lines, used in some places for fall protection on the rear roof, were at least 6 feet from the edge of the roof, exposing workers to falls of up to approximately 17 feet. It was determined that the flags ranged in distance from approximately 2-6 feet from the edge of the roof.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged and supported in such a way that its lowest point (including sag) was no less than 34 inches (.9 m) from the walking/working surface and its highest point is no more than 39 inches (1.0 m) from the walking/working surface:    a) Jobsite, 2205 East Carson Street, Pittsburgh, PA: On or about June 27, 2012, during roofing activities, the employer did not ensure that warning lines, used for fall protection on the rear roof, were set up such that at it's lowest point, the flags were at least 34 inches above the surface of the roof, exposing employees to falls of up to approximately 17 feet. It was determined that the flags in some areas were between approximately 24-30" from the roofline.

29 CFR 1926.405(a)(2)(ii)(E): All lamps for general illumination were not protected from accidental contact or breakage:      a) Job site, 2205 East Carson Street, Pittsburgh, PA: On or about June 27, 2012, during construction activities, the employer did not ensure that bulb covers were present on temporary lighting systems. Employees were permitted to walk/work by a temporary lighting stand that had missing/broken bulb cover guards.
