29 CFR 1910.22(a)(1):  All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:    On or about July 3, 2012, in the facility, employee(s) required to perform metal fabrication operations were exposed to trip and fall hazards due to unprotected air hoses laying across floor passageways and across the top of the mezzanine ladder.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder:    On or about July 3, 2012, in the facility, employee(s) required to perform metal fabrication operations were exposed to an approximate fall hazard of 6' above ground when working on top of Crawler Frames.  Standard railings, or the equivalent, were not provided on all open sides.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored and maintained so as not to create a health hazard to the user:    On or about July 3, 2012, in the facility, employees required to perform welding operations were provided with half-mask air-purifying respirators.  The employer did not establish and implement a written respiratory protection program to ensure that the respirator itself did not present a health hazard to the user.
