On or about June 21, 2012 at a jobsite located at 416 N. Mckendrie Mount Morris, IL, employees were climbing the Alsina Alisply-C form. Employees were using the turnbuckles as steps. The first 3 turnbuckles were spaced 4 feet apart and the last turnbuckle on the top of the form was spaced 3 feet apart.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
