29 CFR 1926.1053(a)(3)(i): Rungs, cleats, and/or steps of portable ladders (except step stools) and/or fixed ladders (including individual-rung/step ladders) were spaced less than 10 inches (25 cm) apart or more than 14 inches (36 cm) apart, as measured between center lines of the rungs, cleats and steps. On or about June 21, 2012 at a jobsite located at 416 N. Mckendrie Mount Morris, IL, employees were climbing the Alsina Alisply-C form. Employees were using the turnbuckles as steps. The first 3 turnbuckles were spaced 4 feet apart and the last turnbuckle on the top of the form was spaced 3 feet apart. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(a)(13): The perpendicular clearance between fixed ladder rungs, cleats, and steps, and any obstruction behind the ladder was less than 7 inches (18 cm): On or about June 21, 2012 at a jobsite located at 416 N. Mckendrie Mount Morris, IL, employees were climbing the Alsina Alisply-C form and were using the turnbuckle as a step. The turnbuckles were approximately 5 inches from the form. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
