29 CFR 1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on were not disconnected from all electric energy sources: Worksite: The employer did not ensure that the 480 volt electrical circuit to which the employee was exposed was not de-energized prior to allowing the employee to perform the work on that circuit. On or about 6/27/2012 an employee received burns after an electrical arc flash that occurred while the employee was working on energized equipment.
