Condition noted on or about 06/29/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301 or equivalent) within seven (7) calender days of receiving information that a recordable injury or illness has occurred: a) Facility Wide - The employer failed to record on their 2012 OSHA 300 Log an employee who experienced a lower back injury on 10/22/12 which involved medical treatment to include removal from work. Condition noted on or about 12/19/12. b) Facility Wide - The employer failed to record all workplace injuries and illnesses on their 2012 OSHA 300 Log within seven calender days. Injuries and illnesses are typically recorded on the 300 Log by the employer at the end of the calender year. Condition noted on or about 10/15/12. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
