Raw meat processing rooms - On or about February 28, 2012, Employees were observed walking underneath elevated loads (meat dumpers) when traveling between the old and new meat processing rooms. The aisles used by employees were not adequately marked to identify a safe passage. 2. Raw meat processing rooms - On or about May 4, 2012, employees were required to walk adjacent to elevated loads (meat dumpers) when traveling between the searing and new meat processing rooms. The aisles used by employees were not adequately marked to identify a safe passage.

Maintenance room - On or about February 28, 2012, employees were required to operate the South Bend lathe with an unguarded chuck. The employees were thereby exposed to the hazard of rotating/moving parts. 2. Raw meat processing room - On or about May 4, 2012, employees were required to walk adjacent to elevated loads (meat dumpers) when traveling between the searing and new meat processing rooms. Employees were thereby exposed to struck-by hazards from moving machinery.
